Shared medical appointments (SMAs)

60–75-minute appointments that include

  • Individual weigh in, blood pressure and heart rate check

  • Brief individual medical visit and physical exam by Dr. Diana Pallin (approx. 5 min)

  • Refills and adjustment of current medications prescribed by NSM providers

  • 30-45 minute group meeting led by Dr. Diana Pallin that includes education and counseling on topics including nutrition, stress management and stress eating, mindset, motivation, weight maintenance, exercise, and other related topics.

  • One free Body Composition (will be performed during one of your individual visits)

  • Support group and connection with other NSM patients who are facing similar challenges

  • Order desired prescription meal replacements and supplements

▶ Recommended number of patients: 7 per shared medical appointment

▶ Patient cost of the SMAs is the same as an individual 15 minute in-office visit

▶ Groups will meet on a regular basis and will be created based on common barriers and areas of needed support

▶ Individual office visits will be necessary in-between SMAs for medical monitoring and review of Body Composition, EKG and lab results. These will complement the shared medical appointments

What to expect:

  • Every participant will sign a Group HIPAA Form and commit to respecting the privacy and confidentiality of the group

  • Each member of a group will have time to share experiences and ask questions

  • Brief intake at check-in to confirm current meal plan, medication refills requests and desired food orders

  • Privacy during vital signs check and individual medical visit

Please note:

  • Body Compositions, EKGs and review of recent lab results cannot be accommodated due to the limited individual time with the provider. These would be performed at a follow up individual medical visit

  • SMAs are only available to established and active patients in our program who have been seen by a NSM provider in the office within the past 3 months

  • New medications cannot be prescribed. These will be addressed during individual office visits to discuss potential risk, benefits and contraindications as well as interactions with any current medications
